In summary only 2 main steps (attn: maid who renew for the 4th year onward don't have to get Fomema examination anymore) :
1)First and foremost get a wangpos or bandraft payable to Fomema Sdn Bhd at RM190 for female and RM180 for male.
2)Then choose the clinic that you want to go for examination from list of panel at their whiteboard.
This can be done at Kompleks PKNS Shah Alam Tingkat 3 (03-55121128/55130581), the longest time spend is actually to queue at post office buying wangpos, wonder why that smart corporation cannot just accept credit card? Once you get the wangpos ready, fill in a form and the fomema registration process should be completed within 30 mins.
Again I reiterate the whole process can actually move online or may be they feel guilty of profitting so much so continue offering the human touch? ;P
Next you will need to bring the maid to the clinic you chose for checkup and xray. Wait for the result in 10 working days and check status at http://www.fomema.com.my/ . Once cleared can proceed to imigresen office.
